Waiver & Release of Liability (Parent/Guardian)
By registering my child/ward for the STYFL Sun Devils Summer Development Camp running from May 27, 2026, through July 17, 2026—held on Wednesdays and Fridays of each week at ALA Ironwood from 6:00 PM to 8:00 PM—I acknowledge and understand that participation in athletic activities involves inherent risks of injury, including serious injury or death. I voluntarily assume all risks associated with my child/ward’s participation.
I hereby release, waive, discharge, and covenant not to sue STYFL Sun Devils, ALA Ironwood, their coaches, staff, volunteers, officers, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child/ward, or to any property belonging to me, whether caused by the negligence of the releasees or otherwise, to the fullest extent permitted by law.
I certify that my child is in good health and physically able to participate, and I agree to follow all camp rules and safety instructions. In the event of an emergency, I authorize camp staff to obtain medical treatment for my child and I accept responsibility for any associated costs.
I understand this waiver is a legal document and I have read and understood it before agreeing.
