1. Cheer Assumption of Risk
I understand that participation in Sun Devils Cheer involves physical activity and carries inherent risks of injury. Activities may include, but are not limited to, stretching, conditioning, running, jumps, dance, cheers, basic tumbling, basic stunting, lifting or supporting other athletes, and participation at practices, games, camps, clinics, and other team activities.
I understand that even when proper instruction, supervision, and safety precautions are provided, injuries may occur. These may include minor injuries such as bruises, cuts, strains, sprains, or falls, as well as more serious injuries, concussions, or heat-related illnesses. I confirm that the registered athlete is in good physical condition to safely participate, and I voluntarily choose to accept and assume all inherent risks associated with their participation in all Sun Devils Cheer activities.

2. Medical Treatment Authorization & Emergency Consent
In the event of an injury, illness, or medical emergency where the parent/guardian cannot be immediately reached, I hereby grant permission to the coaching staff, team representatives, and board members of the STYFL Sun Devils Youth Football Association to authorize necessary first aid, CPR/AED care, emergency medical transportation, and emergency treatment by licensed healthcare professionals.
3. Release of Liability & Indemnification
In consideration of allowing the participant to take part in the program, I hereby release, waive, discharge, and hold harmless the STYFL Sun Devils Youth Football Association, its officers, board members, coaches, volunteers, and facility operators from any and all claims, liabilities, demands, or causes of action arising out of negligence, personal injury, property loss, or damages sustained during participation in any team-sponsored activity.
4. Photo & Media Release
I grant permission to the STYFL Sun Devils to photograph, record, and publish images or video of the participant for team promotions, official social media, and website updates without financial compensation.
5. Parent & Athlete Code of Conduct
I agree that both the participant and family members/spectators will uphold positive sportsmanship, respect coaches, game officials, teammates, and opponents, and adhere to all facility and league rules. Failure to comply may result in removal from team events.
