1. Acknowledgment of Risk I, the undersigned, understand that participation in the STYFL Sun Devils Cheer Stunting and Tumbling Clinic involves significant physical activity and risk. I acknowledge that stunting, tumbling, and jumping involve inherent risks, including but not limited to falls, collisions, and technical errors that may result in serious injury.
2. Physical Condition and Safety I certify that the registered athlete is in good physical health and has no medical conditions that would prevent safe participation in technical stunting or tumbling drills. I understand that athletes will be grouped by experience level to maintain the highest safety standards during high-intensity training.
3. USA Cheer & Safety Standards Compliance I acknowledge that this clinic is led by USA Cheer Certified coaching staff. I understand that this clinic strictly adheres to USA Cheer Safety Standards, including mandatory spotting requirements, skill-progression protocols, and equipment safety regulations. I agree that the athlete must follow all coaching instructions to minimize risk to themselves and others.
4. Release of Liability In consideration of participation in this $100.00 clinic, I hereby release the STYFL Sun Devils Youth Football & Cheer Association, its board, its coaches, and ALA Ironwood from any and all liability for injuries or damages sustained during the course of the event. I acknowledge that this release applies to the use of the facilities and equipment provided at the ALA Ironwood campus.
5. Medical Authorization In the event of an injury, I authorize the clinic staff to seek medical treatment for the athlete if I cannot be reached.
